Escalations that never reach the minutes
When an incident breaks, teams often update a chair by message and “bring it formally next time.” Next time can arrive after the story has softened. Minutes then show calm oversight that never happened.
Good escalation reporting is boring by design: thresholds, owners, a short paper or addendum, and a minute that records challenge and follow-up. The goal is not bureaucracy — it is a reconstructable trail when someone asks what directors knew and when.
If your policy allows verbal escalation in a crisis, say so — and still require a written summary into the next pack. Audits that sample real incidents against minutes expose where the informal path became the only path.
